Welcome to John F. Kennedy Middle School,
My name is Diana Borys (Nurse B) and I am the school nurse. My job is to ensure the safety and well being of your student while here at JFK. If your child has any medical concerns or has a critical allergy to food, insects, or latex, please contact my office. In your welcome packet you will see several forms that are medically related, most of these forms will be green. Please fill them out and return to the school.

The following forms should be returned to the nurse.
- Emergency Illness and Accident form- This will provide the nurse with the necessary information should your child be sick or injured at school. IF you want the nurse to administer over the counter medications like ibuprofen you may indicate this as well.
- Vaccine Consent forms- If your child is 12 years of age or older and you would like them to receive the COVID-19 vaccine at school please fill out the Pfizer consent form, the Big Y consent form and the insurance intake form. This vaccine is free to you and your insurance will be billed. We are highly encouraging everyone who is eligible to receive the vaccine to receive this so that we can keep our schools open and our staff and students healthy.
- COVID Testing Form- The school will have the ability to test students who have symptoms of COVID or may have been exposed to someone who is positive. We will also be offering “routine Covid safety checks”- formerly known as pool testing.
- Medical Release- This will allow the nurse to speak to your child’s doctor if needed for in school support.
- Updated Health Record/Immunization List- This should be provided to the school nurse annually. Please contact your doctor to schedule your child’s physical and make sure his/her immunizations are up to date.
- Medication order forms- If your child needs to take medication during the school day (ie- oral medications, inhalers), have the child’s doctor complete the attached order forms and return them to the nurse signed. Please do not send your student to school with any medications.
